Gwinnett County Senior Services, the designated County Based Aging Program for Gwinnett County is funded by the Older Americans Act, JARC, New Freedom- FTA, State, County and other local funds to administer programs, manage senior centers and senior services programs (home delivered meals, transportation, homemaker services, respite, information and assistance, case management, volunteer and the community living program).
This position is responsible for overseeing operations of our In-Home and Health Services programs for Gwinnett Senior Services. This position will oversee the homemaker, respite care, and Tender Feet programs. The incumbent will supervise all In-home services staff (including Georgia State Certified Nurse Assistants) and will be responsible for oversight of programs, budgets, and contracts associated with the program. The position requires frequent telephone interaction between clients (older adults age 60 and older) and conducts home visits and assessments as mandated by State Policy (DHR Division of Aging Services Section 306 (Homemaker Service Requirements) and Section 310 (In-Home Respite Services requirements). Monitors contracts with outside Agencies providing HMK/Respite and tender feet programs and services to ensure quality assurance and client satisfaction.
Develops and coordinates preventative and/or rehabilitative and other special programs and activities in Senior Centers and for homebound clients. Conducts educational wellness sessions to Community groups including special projects such as Information and Health Fairs and Caregiver Conferences. Coordinates monthly wellness communications to clients through newsletter articles and direct contact. Responsible for all State and County reporting for In-Home and Health Services. The office is located at 567 Swanson Drive, Lawrenceville, Ga.
Minimum Qualifications:
- Bachelors of Science Degree in Nursing or higher level medical degree
- Two years’ experience working with grant funded Senior Services programs.
- Valid Driver’s License
- CPR and First Aid Certificates
Knowledge, skills, and ability should include:
- Budget preparation, review, and grant management
- Two years supervision or management experience
- Problem-solving and mediation
- Ability to handle multiple assignments
- Excellent written and verbal communication skills
- Knowledge of Nursing procedure for working with frail older adults
- Health and well-being assessment principles and practices.

This position will oversee the consolidation, organization, and supervision of differing transportation program components including contractual fixed route services to and from senior centers, two differing types of demand response transportation, and a volunteer driver program for Gwinnett County residents. One of the demand response transportation programs serves adults with disabilities ages 18+ as well as older adults with disabilities, while the other program components are for older adults ages 60+ only. The incumbent will be a member of the Gwinnett Senior Services leadership team and will be responsible for mobility management for Gwinnett Senior Services. The office is located at 567 Swanson Drive, Lawrenceville, Ga.
Minimum Qualifications:
- Bachelor’s Degree in Public Administration, Social Work or related field
- Two years’ experience working with grant funded Senior Services programs and/or two-year experience work with publicly funded transportation programs.
- Valid Driver’s License
- CPR and First Aid Certificates required
